Discharge

Before you leave the hospital, make sure that you and the friends or relatives involved in your care know how to look after you and can accompany you home. If in doubt, ask for treatment programs to be in writing.

Your doctor or health care provider will answer any questions you have. 

If you are assessed as needing support at home, we can organise home care support such as  home help, district nursing, Meals on Wheels, home nursing and rehabilitation.

Check that any follow-up appointments with your doctor or health care provider have been made. Medications which you may require have been arranged prior to your discharge.

If you have problems about returning home, ask the Nurse Unit Manager to arrange an appointment with the Social Worker or Welfare Officer.

Discharge at own risk

You may discharge yourself at any time. However, in doing so against the advice of your doctor, you cannot hold Southern Health or the staff responsible for the outcome. If you wish to discharge yourself against medical recommendation you will be asked to sign a self-discharge form.
