What are Linkages Packages?
Linkages Packages are services that help people remain living at home. We help people who are elderly or who are living with a disability.
Who can ask for help?
You can ask for help if you are:
- over 65 and need a range of different types of care
or
- you are under 65 and living with a disability.
If you are from an Indigenous community, you can ask for help if you are over 45 years.
The packages are available to people who live in these areas:
- Casey
- Cardinia
- Dandenong
What kind of help is available?
Some of the services available include:
- Assistance with bathing, showering or personal hygiene
- Social support
- Transport
- Aids and equipment
- Meal preparation
- Continence pads
- Nursing
Agreeing on the right type of care
The care provided is based on your needs.
You will have a case manager.
He or she will help you choose the right services.
For some people, an Aged Care Assessment will be required. The assessment helps us to work out which services are best for you. The assessment is free.
If you are living with a disability and are under the age of 65, we can help you access services through the Disability Support Register. We will guide you through this process.
Your case manager will discuss all of the options for care with you. You, your family or anyone else representing you may ask questions or suggest changes at any time. Together, you will agree on the right type of care.
Once you have agreed on the type of care to be provided, a care plan will be written.
You will receive a copy of the care plan. This will explain the services you will receive.
Your case manager will make sure that the services are working well for you over time.
Is there a fee?
There are fees for Linkages Packages. The fees vary according to your circumstances. No one will be denied a service they need because of an inability to pay the fees.
Your case manager will discuss the fees with you.
Hours
Our normal business hours are Monday to Friday, 8.30 am to 5.00 pm. An after hours service is available to clients at other times for emergencies.
